The EU-approved intended use of Ellansé is as follows.
Ellansé (a Poly-ɛ-caprolactone-based dermal filler) is an injectable implant indicated for subdermal implantation in the face for the lasting correction of wrinkles and facial aging signs or conditions.
Ellansé has been used at the Androfill Clinic since 2016.
There are several advantages and disadvantages to using Ellansé versus Hyaluronic acid or Fat Transfer Surgery.
The main advantages of Ellansé are:
- It can last up to four years or longer, depending on the model, compared to Hyaluronic acid (1-2 years), and fat transfer surgery (most fat reabsorbs in 9-12 months).
- Ellansé can feel harder and more natural in the penis than Hyaluronic acid or fat transfer.
The main disadvantage we consider important is that Ellansé cannot be dissolved away or reversed using a dissolving enzyme such as Hyaluronidase.
This is a serious limitation of the product, particularly when considering it is used “off-label” in the penis.
We consider any polymer-based (plastic) “collagen stimulators” to be a higher risk when used in the penis than Hyaluronic acid-based dermal fillers.

Firmness — how hard it feels in the erect penis
- When injected correctly, Ellansé is the hardest and most natural feeling of any filler, including fat.
- Ellansé creates new tissue in the penis, which is why it feels natural and firm when erect.
- Ellansé stimulates the production of Type III collagen, which occurs naturally in the penis.
- The rigidity of the product makes it possible to increase thickness beyond a 1-inch gain in circumference without losing erection firmness.
- If Ellansé is injected poorly, its firmness can cause problems — such as a hard, poorly distributed lump.
Durability — how long it lasts
Risk level — probability × severity × treatability of complications
Additional information
Fat transfer: probability of complications high (~25% of cases); severity moderate to severe; moderate complications respond well to treatment, severe ones can require surgery.
Ellansé: probability low (<5% of cases); severity moderate; complications can be difficult to treat as Ellansé cannot be dissolved.
High-quality hyaluronic acid: probability low; severity usually mild to moderate; most complications can be treated successfully with hyaluronidase (dissolving enzyme).
INDICATION:

Important Safety Considerations:
Like all procedures of this type there is a possibility of adverse events, although not everybody experiences them. These adverse events include but are not limited to infection, minimal acute inflammatory tissue reaction (redness, swelling, rash, oedema, erythema, lumps/nodules etc.), pain (which may be temporary or persistent in nature), transient haematoma or bruising.
Other potential adverse events include
– Injection related reactions, include itching, discoloration or tenderness may occur at the site of the injection. These usually resolve spontaneously within several days after the injection.
– As with any implant material possible adverse reactions that may occur include, but are not limited to the following: hypersensitivity, allergic reactions, inflammation, infection, fistula formation, haematoma, seroma, extrusion, induration formation, inadequate healing, skin discoloration, oedema, contusions/bruises, ecchymosis, inadequate or excessive augmentation, loss of correction, interference with local circulation if injected into a blood vessel resulting in vessel laceration, occlusion, embolism, infarction, and abscess at implant site which may result in induration and/or scar formation.
– Rare but serious adverse events associated with the intravascular injection of soft tissue fillers in the face have been reported and include temporary or permanent vision impairment, blindness, cerebral ischemia or cerebral hemorrhage, leading to stroke, skin necrosis, damage to underlying facial structures, nausea and vomiting, lack of ocular movement, significant skin changes, ptosis, exotropia and neurological complications.
– Adverse events, other than mentioned above, could occur as can be with any medical intervention.
