Maximum size increase

Published 4 May 2018 · Updated 21 October 2024

The Short Answer:

Patients should ideally limit the increase in their penis girth to 1 inch, or roughly 20% increase in size.

Going beyond this level will likely stretch and loosen the skin.

The Long Answer:

Maximum volume guidelines have been implemented in the case of Hyaluronic acid and Polycaprolactone based dermal fillers for several reasons, including:

  • Irreversible stretching of the penis tissues.
    • When tissue has been significantly stretched and remains that way for a long period, collagen and elastin fibres become damaged. As a result, they lose some of their ability to retract. In short, should you increase your penis thickness significantly, for example, beyond 20% of its original size, you will likely be left with loose skin should you ever stop using the product and return to your original penis size.
    • Single procedures of more than 10 ml should be discouraged, and patients should be warned of the risk of permanently stretching penile tissues once the total amount of filler they purchase exceeds either 20 ml or a 20% increase from their original size.
  • The limits of Hyaluronic acid to provide lift/firmness.
    • If too much Hyaluronic acid filler is injected into the penis, the erection will likely feel softer. This is not due to any impairment in the ability of the patient to gain an erection but rather the increasing thickness of the Hyaluronic acid gel layer in the penis. Even the hardest of Hyaluronic acid gels will compress if the layer is sufficiently thick. This is sometimes referred to as the “padded penis” effect.
    • A softer variety of Hyaluronic acid filler will cause the erection to feel less firm with as little as 10 ml of filler injected (generally a 10% increase in penis thickness).
    • A very hard Hyaluronic acid filler may adequately support 20 ml of filler and an increase in circumference of 1 inch or more before becoming noticeably less hard in the erect state.
    • These aspects of Hyaluronic acid fillers are called rheological properties; more information can be found here.   Molliard SG, Bétemps JB, Hadjab B, Topchian D, Micheels P, Salomon D.
