Price / Quality

The price of Hyaluronic acid filler varies significantly according to the quality of the product.

Generally, more expensive fillers will feel more natural in the penis (firmer when erect), and will last longer.

A low-cost filler may have a lower price tag, but will not last as long or feel as natural.

Lower cost fillers such as CE Budget should not be used in volumes exceeding 10 ml or they will start to feel ‘squishy’ or ‘padded’ when erect.

MD Volume, Perfecta and Voluma are excellent choices combining firmness with durability.

Repeat Androfill patients benefit from a £10 per ml reduction in filler price.

Recommended fillers for 1st-time patients include MD Volume, Perfecta and Voluma.

These fillers are hard and long-lasting, without being too hard for an inexperienced patient to mould.

Filler Durability – How Long It Lasts2020-08-23T20:46:08+01:00

Filler Lifespan (months)

Durability

Consideration should be given to the estimated lifespan of the filler (half-life).

Hyaluronic acid fillers are safe and reversible; however, they last a maximum of 24 months.

Ellansé lasts over four years, making it the most economical filler on a cost per month basis.

Reabsorption Profile

Hyaluronic acid fillers are metabolised (reabsorb) at a roughly even rate each month (2 – 7% p.m., depending on the brand).

The durability quoted is a half-life, meaning the point at which it is estimated that 50% of the filler will remain.

Hyaluronic acid in vitro reabsorption.

Hyaluronic acid metabolism chart

Ellansé

Ellansé is broken down by the body in a different way to Hyaluronic acid.

Almost all of the effect of Ellansé is present up until the estimated end-life of the product.

Once the active ingredient Polycaprolactone degrades (~4 years), nothing remains to stimulate type III collagen, and the results quickly fade within 6-12 months.

Ellansé in vitro degradation.

Ellanse In vitro degradation

Body fat as a filler. Fat surgically transferred to the penis generally lasts around 12 months before the body reabsorbs most of it.

A small amount of fat may survive for several years where fat has successfully vascularised (gained a blood supply) in sections along the penis.

The penis does not naturally contain fat, making the survival of transferred fat less likely than in other areas of the body such as the buttocks, breasts and face.

Fat Reabsorption Profile

Any transferred fat which has not attained a viable blood supply within a matter of days will be either passed out of the body or form hard calcified nodules of necrotic fat. Successfully transferred fat and calcified necrotic fat nodules may survive for a number of years in the penis.

Filler Firmness – Natural Feeling Result2020-08-23T23:55:38+01:00

Firmness

Filler Firmness

Firmness refers to how hard the filler feels in the erect penis.

Generally, the harder the filler is the more natural it will feel and the longer it will last.

Harder Hyaluronic acid fillers can support a greater increase in total size (more filler) before softness is detected when erect.

Harder fillers tend to be more expensive and can be more difficult to mould smooth.

Even the hardest of Hyaluronic acid fillers such as Ultra Deep will start to feel less firm to some patients in amounts exceeding 20 ml (or a 1 inch gain in circumference). It is advisable to limit the amount of Hyaluronic acid filler injected to approximately 20 ml in total, depending on penis length and depending on filler firmness (softer, budget fillers should be limited to 10 ml).

Budget Hyaluronic acid fillers are generally softer and detectable in the penis even after small procedures of 10 ml or less. Budget fillers can feel even softer than the results of fat transfer surgery.

Ellansé is not a Hyaluronic acid-based dermal filler and can be injected in any quantity without causing the erection to feel less hard.

Ellansé stimulates the production of new tissue in the penis (type III collagen) resulting in a natural, or better than natural texture when erect.

Effective Cost Per Month

Estimated effective price per month

When considering value for money the estimated lifespan (of half-life for HA fillers) must be considered.

At £280 per ml Ellansé is the most expensive filler offered, however as it lasts over 4 years the price per month makes it the best value filler.

The least expensive budget fillers tend to be the most expensive, when considering how quickly they disappear.

Fat transfer is available at the Androfill Clinic for £5,400 + the costs of tests for Covid-19 and bloodwork required for surgery.

Fat transfer is estimated to cost over £380 per month with most patients returning for additional fat transfer surgery every 1-2 years.

(£5,800 procedure price / 12 months) = £483 with an average fat survival rate of 20% at 12 months = £386.67 per month.

Malleability – Difficulty To Mould2020-10-06T00:02:48+01:00

Malleability - Difficulty To Mould

Moulding Difficulty

Harder Hyaluronic acid fillers are more difficult to mould should there be a need to smooth out any initial irregularity in the filler shape following the procedure.

Once a patient returns home, they are encouraged to attain an erection and check how evenly the filler is distributed.

If there is any unevenness, the patient can press into the filler to spread it out before the filler sets firm (after a few days the fill is set).

An inexpensive, soft Hyaluronic acid filler is easier to mould smooth than a firmer and more expensive filler.

More force is required to shape expensive fillers such as Teosyal Ultra Deep and Volux, making them less suitable for first-time patients who have no experience in the moulding process.

See Androfill Instructions section Filler Moulding.

Any imperfection in the result can easily be altered back at the clinic if the filler is a reversible Hyaluronic acid-based product (all fillers except Ellansé are alterable back at the clinic).

CE Budget: A soft forgiving filler, easy to mould smooth.

MD Volume: A firmer filler, however, most patients will have no trouble to spread this filler.

Ellansé: Sets quickly, a moderate force required when moulding. Irreversible so practising with Hyaluronic acid first is strongly advised.

Perfecta / Voluma: Firmer and slightly more difficult to mould than MD Volume. Most patients, including 1st-time patients, will manage to shape these fillers smooth without difficulty.

Volux: Very firm with a tendency to form globular masses. Needs regular checking and re-moulding as necessary every 4-6 hours.

Ultra Deep: The hardest of the Hyaluronic acid filler range.

If moulded successfully this filler provides the best result among Hyaluronic acid fillers and supports the largest increase in size without becoming soft.

Moulding Ultra Deep requires some force to be applied and a high degree of dedication to the process.

It is recommended for repeat patients with experience of the moulding process.

Ellansé Moulding

Due to the relative softness of the carrier gel containing the raw ingredient (PCL), Ellansé is a reasonably straight-forward filler to mould.

However, Ellansé sets more quickly than Hyaluronic acid, and the result is irreversible, making it essential that the moulding process is performed correctly and without delay (on the same day as the procedure).

Patients are strongly recommended to first practice the moulding process with a safe and reversible Hyaluronic acid filler which can be easily adjusted with the use of the dissolving enzyme Hyaluronidase.

Patients wishing to first practice moulding with a Hyaluronic acid filler have 50% of the Hyaluronic acid procedure cost credited against their subsequent Ellansé procedure.

Authenticity - Hologram

Genuine filler purchased directly from the manufacturer can be identified by the hXc hologram on the box.

Price from £199 per ml Parallel imported filler has been purchased from sources abroad (commonly Eastern Europe and Turkey) and is re-sold in the UK. Parallel imported filler must be requested in advance as we do not keep any parallel imported filler in stock. Downsides:
  • The filler cannot be traced back to the manufacturer,
  • no cold-chain guarantee, uncertain storage and handling,
  • the filler is often close to its expiry date,
  • a higher chance of counterfeit filler.
There is a large amount of counterfeit Volux sold in the UK. Counterfeit Volux is commonly sold in the UK and has a high rate of complications. Genuine product sourced directly from the manufacturer has the hXc hologram on the box.

15 ml is the maximum recommended amount per session (in most cases 8-10 ml per session provides the optimum result). Having too much filler in a single session will result in a poor outcome. Ellansé 'L'

(A long-lasting collagen bio-stimulator filler – Up to 4 Years).

Pluryal Volume Filler

Benefits:

Less expensive than HA fillers over the long term as the filler lasts up to 4 years, depending on the model used. Firmer than many of the Hyaluronic acid fillers, and significantly firmer than fat transfer. Ellansé stimulates the production of type III collagen in the penis. Ellansé is reabsorbed and passed out of the body over 4 years, depending on the version used.

Downsides:

Ellansé cannot be dissolved away like Hyaluronic Acid – this makes Ellansé a higher risk filler.

Summary:

The results achieved by patients using Ellansé at the Androfill Clinic have been impressive. Patients report that the outcome feels very natural and firmer than the results from hyaluronic acid. The filler remains a high-risk option as it cannot be dissolved away if there is any irregularity or complication such as nodules or granulomas. The procedure is approved after careful candidate selection and undertaken only by a surgeon qualified to treat potential complications.

Genefill is an excellent product for augmentation of soft areas of the body such as the buttocks, however, Genefill is not safe for use in the penis.

No. Genefill and Hyacorp are not safe for use in the penis. Genefill and Hyacorp are expressly contraindicated for use in the genitals. This means the product is unsafe when used in the penis. The manufacturer of Genefill, BioScience GmbH states the following: (Reproduced in the interests of the consumer). The manufacturer of Genefill has written to Androfill to reiterate that the product should not be promoted for use in the genitals. Dear Sirs, We have noticed that you promote our product Genefill Contour for the indication penis enlargement on your website https://www.androfill.co.uk/penis-fillers/ The use of Genefill contour for breast and genital enlargement is contraindicated. Even if you write that the products cannot be used for penis enlargement it leads to the assumption that it is possible. Herewith we urgently request you to remove this information from your website. A number of inexperienced doctors continue to use body fillers in the penis. Androfill cannot use these fillers because of the risk of complications including Fournier's Gangrene. A doctor might be driven to use Genefill or Hyacorp because the wholesale price is very low. 10 ml of Genefill or Hyacorp will cost a doctor £75, whereas 10 ml of Volux will cost the doctor approximately £1,500 incl VAT. Body fillers like Genefill or Hyacorp are around 1/20th or 5% of the cost of a high-quality filler. A low-cost filler can then be injected in high volumes, for example, 20 or 30 ml, with little regard for the quality of the result or for patient safety. When using a filler contraindicated for the penis, such as Genefill or Hyacorp, the profit margin for the doctor is very high, as is the risk to the patient. If in any doubt, please contact the manufacturer of the product who will confirm the product is contraindicated for use in the genitals. Yes. CE Budget is approximately the same price as Genefill, however, it is safe to use in the penis and will not feel as soft as Genefill.
The Androfill Clinic is able to dissolve Genefill, Hyacorp or any other body filler which has been injected into the penis for approximately £100 using Hyaluronidaise enzyme.

15 ml is the maximum recommended amount per session (in most cases 8-10 ml per session provides the optimum result). Having too much filler in a single session will result in a poor outcome. Having too much filler in total (typically more than 20 ml at any one time, depending on penis length) will result in a poor outcome.

Fat Transfer Surgery

Fat Transfer Surgery £5,400

Fat can be transferred to the penis instead of dermal filler.

£5,400

Filler Attributes - Stomach Fat

Firmness 35
Difficulty to Mould 80
Long Lasting 50
Effective cost p/m £ 387
Highest Risk
  • A small amount of fat might last many years (Calcified nodules and encapsulated fat necrosis)
  • Liposuction of fat from the stomach included
  • Soft, 'fatty' layer of fat surrounding the penis - erections will feel softer
  • 'Top-Up' procedures from 12 months and at bi-annual intervals thereafter (depending on fat reabsorption)
  • Can not be dissolved away or reversed
  • Complications may be difficult to treat
  • Highest Risk
  • Requires Penlie Augmentation Surgery

Fat to be transferred to the penis.

Liposuctioned Fat

Benefits:

  • Although most of the fat will reabsorb in 9 to 12 months, some of the fat will last a long time, particularly hard lumps of necrotic fat.
  • A liposuction is included as part of this procedure enabling you to remove a small amount of fat from an unwanted area.
  • Commonly included free as part of Length Plus Girth surgery.

Downsides:

  • As there is no fat in the human penis to begin with, fat does not reliably revascularize (form a new blood supply in order to survive).
  • This can leave areas of the penis in which fat has survived, and areas in which it has not, causing some unevenness.
  • A layer of fat surrounding the erection is sometimes described by patients as feeling soft or 'fatty'.
  • It is likely that a patient will need 4 or 5 fat transfer surgery Top-Ups over a number of years in order to maintain the level of fat in the penis, and smooth out any irregularities.

Issues with fat survival / lumps of fat.

Fat Lumps

Summary:

Transferring fat to areas of the body where fat naturally exists can give positive results, such as in the case of Brazilian Butt Lift surgery.

However, fat is less well suited to the penis, in part due to the texture of fat (how it feels), and the absence of surrounding pre-existing fat into which to integrate.

This is why most Urologists do not approve of fat transfer to the penis.

Filler Types: Dissolvable (Hyaluronic) vs not-dissolvable (Ellanse).

Hyaluronic acid filler (H.A.)

(Lower Risk)

Hyaluronic acid is a naturally occurring substance made by the body.

H.A. fillers are generally very safe (used commonly in the lips and face), and have a low complication rate.

H.A. fillers are biocompatible and can be dissolved away (reversing the procedure if necessary).

  • Safer (Lower Risk)

  • H.A. is naturally present in the body

  • Used for 15 years in the penis (South Korea since 2002)

  • Reversible (safety aspect)

  • Able to be dissolved (Hyaluronidase)

  • Alterable after final result

  • Low complication rate

  • Can be used in Glans Penis (Penis Head)

  • Smooth (with Androfill technique)

  • Firm (with Androfill technique)

  • Lasts 18 months (on average)

  • Budget £1,000 – £1,500 per year (10 years £10-15 k)

  • Main complications: Infection (low risk when taken with antibiotics), granuloma possible (rare).

Hyaluronic acid fillers are safe and effective for penis augmentation.

Ellansé filler

(Higher Risk)

Ellansé is a polyester comprised of Polycaprolactone microspheres.

Ellansé encourages the production of new collagen. It is biodegradable and slowly leaves the body over a number of years as it breaks down.

Once Ellansé is injected, it cannot be dissolved away or altered, which makes it higher risk than HA.

Ellansé

(Biodegradable Polyester / PCL)

Ellanse E Filler

  • Less Safe (Higher Risk)

  • Artificial Substance (Polyester / PCL / Plastic)

  • Used in the penis for 2 years (since 2016)

  • Irreversible (the filler can’t be dissolved away once placed)

  • Non-alterable (the substance remains for 4 years)

  • Can’t be used in Glans Penis (Penis Head)

  • Smooth (with Androfill technique)

  • Firm (with Androfill technique)

  • Can last 4 years (Ellanse ‘E’)

  • Less expensive per year (if no complications occur)

  • Main complications: Granuloma (hard lumps forming in the penis), infection, other tissue reactions.

Treating a complication caused by Ellanse can be expensive and may cause permanent penis damage.

Due to the higher risk of complications, Dr Horn prefers Hyaluronic acid fillers to Ellansé.
